To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. Dr. Livaditis does not participate in any “DMO” (“HMO”) type insurance programs. Our staff will work with you to process claims for standard plans or out-of-network PPO insurance plans.

Please call if you have any questions or concerns regarding your initial visit or dental insurance.

Please bring your insurance information with you to the consultation visit so that we can expedite reimbursement. Fees for the initial visit and all one-time consultation visits are due and payable at the time of the visit. Insurance claims will be submitted and payment will be assigned to the patient.

We encourage submittal for pre-determination of benefits by your insurance carrier whenever possible.
